Case Manager

Sierra Forever Families is a private non-profit foster family/adoption agency that focuses on finding permanency for children and youth in the foster care system. We are seeking a creative, enthusiastic and passionate individual for a full-time position as a Case Manager in our Permanency Support Services (PSS) program in our Auburn office.

Position Summary

The PSS Case Manager is a member of the interdisciplinary youth permanency team that utilizes best practices and innovative techniques in finding permanency for youth living in foster care. The PSS Case Manager will provide in-home and community based supports and case management to youth in assisting them to achieve permanency. The chosen candidate will be responsible for EPSDT Medi-Cal billing and Placer County Children’s System of Care documentation requirements.  

Qualifications

  • Bachelor’s degree in Social Work or Social Services required
  • Ability to effectively engage and work with youth in foster care
  • Understanding of loss and grief and youth development
  • Working knowledge of mental health issues and community resources
  • Ability to work collaboratively and effectively with other team members, county child welfare agencies and community resource agencies
  • Knowledge of Medi-Cal billing and procedures desirable
  • Willingness to work non-traditional hours as the job necessitates
  • Understanding of basic wrap around community-based program principles
  • Basic knowledge of child welfare regulations
  • Experience in foster care and special needs adoption preferred
  • Proficiency in computer technology
  • Willingness to work non-traditional hours as the job necessitates
  • Cultural competence to work effectively with diverse populations
  • Possess a valid California driver’s license, provide proof of adequate vehicle insurance 250/500/100k and have access to reliable transportation
  • Must submit to and pass health screen, TB test, criminal record and child abuse index clearances

The ideal candidate will have a passion for youth permanency and be a creative and effective individual able to develop trusting relationships with youth living in foster care as they strive for permanency in their lives. In addition, they will be a creative and effective solution oriented individual who is comfortable working in a highly collaborative environment within the agency and with community partners.

We are seeking a creative, enthusiastic and passionate social work professional for a full-time position as a Youth Permanency Worker (YPW) in our Destination Family program in our Sacramento office.

Position Summary

The Youth Permanency Social Worker (YPW) is responsible to utilize mental health, social work best practice and innovative techniques in finding permanency for youth in foster care. The chosen candidate will be responsible for EPSDT Medi-Cal billing, assessments, treatment plans, CANS and other documentation requirements. The position will require case management skills in the coordination of services with the other members of the Destination Family Team.  The YPW is part of an interdisciplinary team that includes a clinician, Parent Partner and Youth Skills Trainer.

Qualifications

  • MSW, or Master’s Level degree in a related field
  • Licensed or license eligible preferred
  • Knowledge of Medi-Cal EPSDT billing and documentation
  • Familiar with the affects of loss and grief and youth development
  • Experience working with children and youth in foster care
  • Working knowledge of mental health issues and community resources
  • Social work experience in foster care and special needs adoption preferred
  • Knowledge of child welfare, community care licensing, adoption and foster care regulations
  • Strong practical and theoretical foundation in strength-based practices
  • Knowledge of wrap around and community-based program principles
  • Ability to work collaboratively and effectively with other team members, county child welfare agencies and community resource agencies
  • Proficiency in computer technology
